APPLICATION REQUIREMENTS

In order to apply, the applicant should appear in person at the visa application center or Consulate.
It Personal appearance of children is also required.


Applications of individuals under the age of 18 years have to be signed by parents or a person exercising permanent or temporary parental authority or legal guardian. Adults and children who are at least 12 years old are required to submit fingerprints.

In accordance with Art. 23 of Regulation (EC) No 810/2009 of the European Parliament and of the Council of 13 July 2009 establishing a Community Code on Visas, in order to obtain a National visa applications should be submitted within a period of no less than 15 days before the scheduled departure.

 

 

I.  Passport validity

 

The passport should be valid for at least 3 months after the date of intended return from Poland. Passports issued more than 10 years before the date of application shall not be accepted. The passport should contain at least 2 blank pages for visas. Passports are accepted without additional wrappers, ex. plastic or leather. Previous passports should also be presented by the applicant (original and photocopy).

 

 

II.  Requirements for photo

 

Photography sized 3,5x4,5 cm, taken within the last six months, on a white background, with good focus, clearly presenting the eyes and face from both sides from the top of the head to the top of the shoulders, so that the face occupies 70 - 80% of the picture. The photo should show the person without glasses, looking straight ahead, with open eyes, not covered by hair, eyebrows must be completely visible, with a natural face expression and closed lips.
The person being photographed cannot have headwear, except for the cover worn by members of a religious community after the submission of certified proof of affiliation to it (ex. Identity card). Also in these cases the shape of the face must be exposed. In the case of people wearing glasses, the photograph must meet the following criteria: non photochromic or adaptive lenses, the frame cannot cover any part of the eye, light reflections should be avoided. Contact lenses may remain, provided that they do not alter the natural colour and shape of the eyes. Photograph should be signed gently (so as not to destroy the image) on the back (first name, last name and date of birth). In the case of children who are under 5 years old derogations from the above principles are permissible.

 

 

III.  Additional information


 

  1. Visa application form
     
    The form must be filled in Latin letters and points 36 and 37 must be signed. Minors shall submit an application form signed by a person exercising permanent or temporary parental authority or legal guardianship.

  3. Travel Medical Insurance
     
    Medical Insurance with a minimum coverage of €30 000 valid for the whole period of residence in Poland. If you apply for a work visa - the insurance policy should cover at least first month of stay in Poland or the applicant should prove that has medical insurance in Social Insurance Institution (ZUS) in Poland. In the case of applying for a “Study, education, research visa”, “Erasmus +”, “Volunteering” - the insurance policy should cover the whole period of residence in Poland.
     
  4. Minors
     
    In case of a minor (under 18) traveling alone or with one parent requires the consent of the other parent or legal guardian (original and photocopy). When obtaining consent is impossible - documents explaining the reason for the lack of consent (single parent certificate or other documents). When a minor is traveling alone, the consent of parents or legal guardians for travel abroad expressed in notarized certificate, must be submitted.
    Visa application form must be signed by a person exercising permanent or temporary parental authority or legal guardianship. The signature should be the same as on the enclosed copies of the passport.
    The application should include a copy of the passport (travel document) of the parents or legal guardians containing personal information with photos and in case of having a Schengen or National visa of a parent or guardian, their photocopies.
    Minors shall provide proof of financial means by the parent, by presenting the sponsorship letter.

D Visas – Required Documents


Documents requested at the visa application:

 

  1. Application form completely filled (in Polish, English or Turkish) and signed by the applicant.
  2. The separate application for each child with photograph in case of a journey with a child being registered in parent’s passport.
  3. Two biometric photos (no more than 6-months old, 35–40mm in width, close up of your head and top of your shoulders so that your face takes up 70–80% of the photograph, in sharp focus and clear, of high quality with no ink marks or creases, show you looking directly at the camera, show your skin tones naturally, have appropriate brightness and contrast, be printed on high quality paper and at high resolution, show your eyes open and clearly visible - no hair across your eyes, show you facing square on to the camera, not looking over one shoulder (portrait style) or tilted, showing both edges of your face clearly, be taken with a plain light-colored background).
  4. Original passport issued within the previous 10 years and valid at least 3 months beyond the end of the intended visit, that must contain 2 blank pages to ensure that there is space to affix entry/exit stamps for visa holder.
  5. Any other international passports must be attached to the application (the original and a photocopy).
  6. Documents confirming the purpose and conditions of the planned stay in Poland.
  7. Proof of sufficient financial means available to the applicant to cover the travel costs can be provided in the form of: a bank account statement for the last 3-6 months, signed traveler cheques, credit cards (letter from a bank confirming that the credit card account has funds), an employment certificate confirming the position and salary during the last 6 months, an invitation (the original and photocopy).
  8. Proof of adequate travel medical insurance covering the entire period of intended stay, valid for all Schengen States and covering any expenses which might arise in connection with repatriation for medical reasons, urgency medical attention, emergency hospital treatment or death during the stay. The minimum coverage must be 30.000 EUR.
  9. If the applicant is a minor and travels alone or with one parent, a consent of the other parent or guardian is required (the original and photocopy), in cases where such consent is impossible to obtain - documents explaining the reason for the lack thereof (a statement certifying their single parent status or other documents). Birth certificate (the original and photocopy). A copy of parents' or the guardian's passports and a copy of valid visas in the case of joint journey.

The following documents shall be presented by either of the categories:

 

For work:

 

- authorization for employment in the territory of the Republic of Poland issued by the Provincial Governor competent for the employer's seat in Poland,

- valid extract from the National Court Register, confirming entry in the register of entrepreneurs of the company that is to employ a foreigner,

- documents certifying professional education and qualifications to perform the job by a foreigner at a position mentioned in the permit to work.

 

For undergraduate / master's or doctoral degree courses:

 

- the original document authorizing to study in Turkey,

- certificate of enrolment issued by a Polish university (including the current year and semester),

- confirmation of payment of the tuition fees,

- documents certifying financial capability to cover the costs of stay in Poland.

 

For participants of academic exchange programs (e.g. ERASMUS):

 

- certificate from the home institution sending a student as a participant in an exchange program with the name of the exchange/scholarship program, the names of the Polish host college, exchange period and the amount of the scholarship,

- certificate from the home institution accepting a student as a participant in an exchange program with the name of the exchange/scholarship program, the names of the Turkish college sending the student, exchange period and the amount of the scholarship.

 

For Resident Permit:

 

- the decision of the competent Provincial Governor to grant a residence permit in Poland for a fixed or indefinite period.
