Visa - for Work Purpose

Waiting time for obtaining visa is 8-15 days.

REQUIREMENTS:

1. Visa application form duly filled and signed by the applicant.
2. 2 recent photographs of the applicant, not older than six months.
3. An ordinary passport or other recognized travel document valid for at least three months from the expiry date of the visa applied for. This document must have at least two empty pages. Please bring previous passports, if any, and attach with application copies, on A4 size paper, of used pages.
4. Photocopy of your national ID card.
5. Third country nationals residing in Pakistan must provide supporting documents proving legal residence.
6. If you have been refused a visa by an Embassy or High Commission in the last two years, written explanation about reason for trip and refusal.
7. Formal "work permit" issued by Urząd Wojewódzki in Poland on the request of an employer.
8. Applicant’s work experience certificates, school diplomas, professional skills certificate.
9. Certificate (NOC) from last workplace in Pakistan.
10. Photocopy of confirmed airline reservation showing dates of departure and return.
11. Original  AND copy of a travel medical insurance policy of not less than 30,000 Euros. The policy must cover medical and hospitalization expenses on Schengen territory as well as repatriation costs for at least first 3 moths of  stay or until the first payment of the health insurance tax by the employer. Consult valid list of authorized insurance companies  in Pakistan as agreed by Schengen Partners in Islamabad.
12. Original bank statements of the last six months.• National Tax Number Certificate (NTM) for the previous two years and proof of income tax payment (FBR).• Last three salary slips.
13. Proof of accommodation.
14. Proof of health insurance for the whole period of stay in Poland or travel health insurance policy.   
15. The Consulate might ask for additional documents if found necessary.
